STEP 2 - CONFIRM YOUR ENROLLMENT
Contact us to confirm your intent to enroll.
The enrollment fee will be due at this time to secure your spot ($100/child or $150/family with more than one child)
STEP 4 - COMPLETE ENROLLMENT FORMS, INCLUDING HEALTH RECORDS
Forms can be dropped off a the center, emailed to email@example.com, or faxed to (866)330-6122
Important Note: Enrollment paperwork, including the healthcare summary and immunization forms completed by your child's healthcare provider, must be received at least one business day prior to your start date.
